10 days is not a lot, since you have must do for lake comp, cinque terra, I would concentrate on the northern or central part of Italy, leaving sorrento and Amalfi coast for the next time.
Florence is a must, Sorrento and Capri could be cities to add to the list. Pompei if you’re also into history. I didn’t particularly enjoy Rome due to crowdedness but would recommend to see the Vatican.
Listen an Italian…go to Positano and Capri (3 days)!

I’d skip Milan entirely. Hell, skip Lake Cuomo! Listen to D2 and spend some time on the most beautiful coast - the Almafi Coast - over Cuomo. Florence is an absolute must too.
Maybe Venice? Or Florence?
Not a whole lot of room for 10 days. I’d suggest a mid way point as your third destination but I wouldn’t push for more than 3 destinations. Florence would be a good choice.
I would recommend Venice or Florence for a few days. The Amalfi Coast is beautiful but it doesn’t sound like you have enough time to add that to your itinerary.
Depends if you’re looking to see historic sights and go to museums or are mostly interested in the coast for the views and charm. I think everyone needs to see Rome, Florence, and Venice in their lifetime but it is a lot to pack in and I think you need minimum 2 nights each for Rome and Florence. Skip Venice this time if on a budget or if you don’t like crowds (it’s cold but empty in winter). Reserve Vatican and any other museum tickets online in advance! They can sell out and lines for some museums can be hours long!! If you only go to 1 museum in Florence make it the Uffizi and also walk around the Duomo (of course!).
Lake Como is beautiful but there is not much to do there at all, it’s where people go to park their yachts and escape to their mansions and if you don’t have either I would just do it as a day trip. If staying in Milan. A day in Tuscany in a small town would be nice in between Rome and Florence if you’re into wine. Bologna is also underrated since it’s basically the food capital of Italy and has the oldest university.
I would only go to Cinque Terre by ferry because hiking stairs uphill in 85 degree weather plus humidity isn’t my idea of a good time.
I like Sorrento/Positano and Portofino more than Lake Como because there are more shops and restaurants and the weather will be hot if you want to lay on the beach and swim in the ocean in September. For that I would also recommend Sicily (lots of cool churches to see in Sicily too, but expect it to be hot and tropical).
Fly into Milan and take the direct train to Varenna (2 nights).
Take the train back to Milan and transfer to Cinque Terre. Vernazza and Manarola are the prettiest towns (3 nights).
Train to Florence (3 nights).
Train to either Venice or Rome, stay 2 nights and fly home. I’d probably recommend Venice as one full day is barely enough to scratch the surface in Rome.
Appreciate the recommendation! Flying into Rome so maybe will try something like this in reverse (Rome -> Florence ->Cinque Terre -> Como -> Milan)

Flying into Rome and out of Milan with the hopes of hitting Como on the back end. Looks like the overwhelming response is Florence so hopefully can fit that in on the way to Cinque Terre
Also if you do goto Florence, make an early rezzy at La Giostra. Well worth the visit, one of my best meals in Italy.
