Sc1 is right. You may be able to appeal and get it waived or reduced. Also sue EY for additional severance, citing the difficulty of obtaining a new job in the pandemic. Ask your lawyer to negotiate more months of severance.
This is bad advice. What are the grounds for suing here?
Sorry to hear OP. You are the first person on a visa from EY I know of that has been laid off. I thought they would go after people on visas as a last resort. In any case, I wish you all the best - If you choose not to pay and go home, you will have a trashy credit rating if you ever come back but nothing that can't be fixed over time. Do what you need to.

If you don’t come back to the US in 7 years or ever there’s nothing collections can do
I mean it’s a different country it’s not like the US can arrest you for a few thousand dollars of debt if you don’t care about your credit score for 7 years. One of the few types of debts that don’t discharge is student loans, although I suppose someone can turn it into another type of loan that does discharge in bankruptcy or fall off your report

I think the amount gets sent to collections and your credit rating drops like anything, so if you ever come back, you’ll be in deep trouble

You can call the hospital and tell them you got fired. I’m pretty sure they will reduce the amount to 1-2K.
Talk to the hospital, I have seen things getting reduced even 50 percent or more. Tell them your situation

You can always apply for waiver. Every hospital has this option, they don’t tell you. You have to ask them. I might be wrong with the terminology but there will be an option for sure.

Also I didn't know EY was laying off people, didn't see any LinkedIn posts come through like Deloitte.
They are being smarter about this and not announcing it like DT did, but they are slowly laying off people in every office but they call it “performance separation”

Try to find an amicable solution would be my advice. As many have suggested on this thread, try to negotiate a discounted payment or waiver. But why exit on bad terms? I totally understand the disappointment associated with getting laid off. Nevertheless the country did offer you a life here. The health system is unfairly priced but it is the way it is.

First negotiate for reduction in the bill and then pay the remaining bill immediately by credit card, and then transfer to another card offering balance transfer with reduced interest. Sometimes they offer 0% interest for 12-18 months if you pay the balance transfer fees usually 3% of the amount.
So 6500x.03= $195 balance transfer fees can give you more time (12-18 months) to pay off the amount.
Check all your credit cards for balance transfer offers.
There are many credit cards out there right now which have balance transfer offers.
I guess if you leave and default can they deduct from 401k? If you leave it to liquidate later?

The insurance companies doesn't do squat, cuz they also ready paid "80%" of the deal they had signed up for.

If you are prepared to leave, tried asking for a job in India? Atleast you have something when you go back? If visa is valid for longer time, try renting the house so you could come back?
Sorry to hear OP, just consider what might happen if tons of H1Bs started to do this. You could be part of why the system gets more restrictive for future H1Bs. I mean, this is how many existing restrictions came about. Best wishes to you and hope you find an amicable solution that others are suggesting. You can definitely negotiate.
