Positive pay decisions - self approval. Wouldn’t this be a fraud risk? An open opportunity for the employee to cash a check & retire in a country with no extradition treaty? Or am I overthinking this?

Depends what are the limits and controls in place surrounding it. If there really is nothing and the preparer can approve it themselves without oversight then that’s pretty bad.
