“Pound for pound” is an obvious figure of speech. How pedantic do you have to be to assume OP literally meant the weight of the employees?

I dont think it matters really - I have found amazing talent at big agencies ... mid-sized and a few small agencies are golden nuggets with focused talents.
The difference is, when you tap one of work, which team they give you - always determined by how much money they spend and/or if it gives a great spotlight on the work - ie. Cannes or Clios. Usually, one average sized company without a $10M budget gets a great team, but not the superstars who are on the pitch - no disrespect, I was on the C team at my agency decade ago, but were hungrier for the work. Did we win awards? no. But that work is still lives today in the company's GTM strategies. If you need big conversation changing work? Look for the right relationship - not the agency. Also, each Agency specializes in certain work. So make sure you're doing your homework.
I suppose its not the answer, but pound for pound, some of the best work I have achieved is having my own in-house team with agency experience, and augmenting work - like national campaigns - with an agency who can both produce and distribute the campaign.... and usually, its a mid-sized or small agency with a small, but mighty team - who are eager for the brief. Some of the worst experiences I have had are with the Large agencies so seemingly are bloated and I usually offset that work with either my in-house or one of my small gem agencies who will charge significantly less.
It's a balance, really.
