Recruiters constantly reach out to me on LinkedIn, ask for my CV and say my ‘experience fits perfectly for X role’ - then they disappear into the void after that. What is with that?! Should I reexamine my CV or just put it down to another recruiter filling in their quota of hires they had to present to client?

I realize it may vary by company, but does putting a referrers work email (with their knowledge and permission) on the relevant portion of the job application help increase the chances that your application is seen?

Usually it's the part of the application where it asks how you heard about the position and if you select the "company X employee" option, it will ask for their company email.
