Be exhaustive. Highlight all the cases you billed a decent amount of time to, and the work you did on each. This is what the partners are going to use to write your evaluation, so don’t short change yourself. It’s annoying, but worth the time.
I personally don’t write about anything bad in my self-evaluation. For example, sometimes I struggle with time management issues (thanks ADHD), but I will never put that in writing myself. I’m in a litigation-based group, so my goals are always things like continuing to get more court/deposition experience or more experience supervising juniors, etc.—nothing that would acknowledge a performance issue. I always plan to address any issues raised by the partners during the verbal review because I don’t want to give them ammo to write about… (Granted, my reviews are generally very good, so if you made a huge mistake, you may need to preempt that in writing.)
And one tip: Save a copy of your self-evaluations so you can partly reuse them in the future.
