Not to be a Ted Mosby but the term is Big 4 and refers to the four largest global professional services firms (Deloitte, PWC, EY, KPMG) that arose after market collapse around 2002. The three largest consulting firms are McKinnsey, BCG and Bain. Not sure where the term big 5 came from but if you try and Google Big 5 consulting or accounting firms the results are about the same.
OP are you worried about explaining why you’re leaving after only 5 months? I think people would understand not wanting to live the travel life, especially for health reasons. (And if they don’t, maybe they wouldn’t be a good fit for you?)
Are you thinking the 5 month gap is how long it will take you to find a new gig? Because that’s no problem- I took 4 months off while applying for new jobs after leaving my last job, and no one at my new job blinked when I joined
Why exclude? That will be deception.....do you mean the big 5 ....BCG, bain, mck, pwc, Deloitte, and ey? I count six
You have to include the job that you are currently in if that’s what you are asking. If it’s a gap between this job and your next, it shouldn’t be the end of the world to have a short gaps I’d be a little weary given the market right now to leave without having another job.

Perhaps there's a lesson to be learned here about asking the right questions before taking a full time position. I thought everyone in Big 4 knew to expect minimum 25% travel. Travel expectations should be included in one of the worklife balance questions you ask your interviewer. Also, I would argue that a person that leaves a job 5 months in because they didn't fully vet the position is probably not the best candidate for a job.
I just assume anyone who uses the phrase "Big 5" works at Accenture...?

OP, life is too short to be miserable. If it’s not working, I’m sure there’s something out there that is a better fit for you. Start the search and worry about the explanation later
Update: I lined up a crap ton of interviews and decided to talk to my practice (we are a new and growing practice) lead in full transparency. He was super understanding and accepted my proposal for reduced travel for this project and anything else that I work on under his practice. We also talked about ways I can contribute to the practice both on client facing and BD work. Cheers!

Talking to other boutique firms, industry, and tech companies. Do you guys think that a 5 month gap in my resume will hurt in the long run? Anyone else ever go through a similar experience and willing to share thoughts/lessons learned?
I think I can find a new gig before the year ends, there would just be a gap in my resume for my time spent here that I might just exclude from my resume.
Also, what’s big 5? MBB + Big 3 = 6, apparently KPMG been kicked out of Big 4 😳
Yup, I understood the travel expectations but didn’t fully grasp the impact on my life. Also had a few traumatic life events happen since I started and the travel isn’t helping with the healing. Thank you all for your advice! Very helpful.
