That does sound like depression, but it could also be a health issue. Schedule a physical with your doctor, tell them what you’ve been struggling with, and get some blood work.
And for reassurance, I’ve discovered a lot of people with adhd struggle with things like showering and grooming routines, because of a mixture of executive function issues and sensory sensitivity.
There are a few things that can help when struggling to get out of bed and maintain personal hygiene. First, try to set a routine for yourself and try to stick to it as much as possible. This can help your body get into a routine and make it easier to do the things you need to do everyday. Secondly, try to stay organized. This will help reduce stress and make it easier to find what you need when you need it. Lastly, make sure you are taking care of yourself mentally and emotionally. This includes taking time for yourself, relaxing, and doing things that make you happy.
Been dealing with this a ton lately in regards to getting out of bed...I've found that physically exhausting myself helps me stay asleep, so plenty of exercise and sunshine help me stay down at night. Try meditating or considering things your grateful for too, that always helps me.

Are you staying up too late? Do you exercise? I exercise in the morning. Every day. Only thing that gets me moving.
No not really I sleep at reasonable hours. Just can’t get up.
I think I’d also recommend getting your vitamin levels checked out. It can easily be a deficiency
Yup those two will definitely do that to you. Obviously check other avenues but that should be an easy fix

Sleepytime tea helps me out a lot. Traditionally really relaxes my body. Sometimes I even have two cups, I don't even care if it's placebo. I hope this helps!

Consider investing in a sleep app that helps you wake up based on your sleep cycle. Often times our time blindness contributes because our "internal clocks" really are "off"
