I just left a small company (had 10 employees). During the first year, I got 2 weeks PTO, 1 week paid sick, and health insurance completely paid. That same employer also contributed 275 dollars to our HSA account every month. No dental or vision. Had a simple IRA with a 3% match. It really just depends on the company. I’ve seen smaller companies offer better benefits than larger companies but some simply can’t afford to. Depends on their revenue, the President, etc.

1 week PTO for the first year and then 2 weeks every year after, 3 sick days, and 12 paid holidays. We have primary care insurance (which doesn’t pay for much except virtual visits), dental, and vision. We have every other Friday as an on-call day and 3% Simple IRA match.
