Has anyone ever done that? Sure. Where are they? If the client found out and had an issue, at a minimum probably fired. Just send them an email and say you have an additional document for them to sign. It’s not even a big deal. Never ever ever ever ever sign something for your client without express written unequivocal consent.

Do DocuSign or if logistics are an issue you could also offer up, via email, to conform (with their permission) their signature and append on X document and have them reply with yes. And then save to the files.
We have clients that we keep their signature on file. But we always send them the sig pages and confirm that we can apply their signatures.

Friend’s father was disbarred for doing so as a witness to a will. Got his license back 20+ years later.

I have a classmate that worked in BIGLAW in the early 1990s and claimed closings were like editing the high school newspaper, with signatures copied, cut and pasted into documents as needed. But, he told a lot of tall tales so he might have been making it up.
Now, it would not seem very hard to get another signature electronically so you should be able to avoid this problem.
A lot of documents need original, wet signatures so the electronic thing doesn’t always work. I wish it did.
