Tell me more, I may need personal insurance (paid by my employer, since we have no other US employees) and whenever I look at quotes I’m like wtf?
The premium rates depend on age and height/weight ratio. The medical history part is only for qualification purposes and doesn’t affect premiums.
This insurance is based mostly on medical qualification. With this being either your height and weight are in the specified range or you do qualify medically by not having certain pre-existing conditions. This is what makes it “cheaper”, the qualification is quite specific regarding history which means not everyone is available to obtain this coverage.
The co-insurance is also in-network. You have the option to choose your amount and it is valid up until your out-of-pocket max is reached. But don’t be fooled, you do receive many many benefits before you have to satisfy that deductible.

The only real “catch” is medical qualification. Being within an applicable height/weight range, and not having certain pre-existing conditions.

This does not appear to be health insurance at all. It's a fixed indemnity plan with annual and lifetime maximums. Meaning that if you have an expensive medical problem, you're not protected.
There are different options when it comes to the policy maximum. Please reference.
