Thankfully, I never have in 25 years of working with children. Never hurts to be prepared though.
I had a first grader choke on a hamburger and had to do the Heimlich to get it out. I had a kindergartener have a seizure and went in the ambulance to the hospital. We couldn't get in touch with the parents so I went with the child and stayed until parents got to the hospital. Then I had another teacher pick me up and I went back to school. Very scary day for that one, but I did get to explain to kindergarten that their friend was fine and would be back to school soon.

I've had to stay with a child until the nurse came more than once, because I am CPR certified I did. After 4 years I have not ever used it.
