I've done it. Just be completely honest. Everyone makes mistakes. They were very understanding and I'm so glad I went back and asked for another chance.
What's the worse they can say, no?
There is no offer if you rejected it. Companies don't present standing offers. And why would a firm want you if you originally declined. If you owned a company, what would you think of someone like you who doesn't know what they want? If anything, they'll be glad they dodged a bullet with you.

Zero. At the associate level nobody is going to put up with this one month lag. That position is already gone.

If the job were still available that would be one thing, but as everyone said, it's unlikely to have stayed unfilled unless they were looking for a very specific skill set.
