Direct method is the best, don't beat around the bush.
Assume good intent. Be respectful.
Give them space to do the right thing. Ask a colleague to go with you as a witness and to offer support. A union rep is good.

All of the above suggestions are great. The only thing I would add is that in some cases, if you have a potential solution, offer that too. Oftentimes when I have a problem, I have an idea of how it can be solved. Sometimes my principal agrees, sometimes she doesn’t. But I think she appreciates the thought at least.

I think it depends on the topic of the discussion. But asking for advice on how to handle something is always good.

And take a Union rep if you're going to discuss anything that might remotely involve possible discipline or a possible grievance.

Ask colleagues ahead of time how they would approach the problem with whichever admin you need to report to.
Also, as cheesy as it sounds, there is real value in the “I feel” statements for both you and admin. It helps remind everyone that there are always multiple views and angles in every situation. It also allows people to correct misunderstandings and offer contradictory viewpoints without contradicting people directly.
Find something positive to say first and thank them for the opportunity to serve the students.
Depends on the conversation, but in general just be direct.
I recently was having anxiety when it came to my work performance and I couldn't handle it anymore. So I went straight to my boss and just asked how I was doing and everything turned out great!
My best advise is: be kind, be respectful, be on-time if it's a scheduled meeting. Also, don't make a mountain out of a mole hill if it's a small problem!

Be sure you drop by to discuss the good things too. Admin is more open to "I have a problem I'd like to discuss with you" if you already have a good rapport.
