I view this as I view any medical decision. We, as physicians, have the obligation to advise and guide, but at the end of the day, it's each person's individual right to make their own medical decisions and we should be supportive of that.

Responsibility is to be educator, advocate, clinician. Above all, Do no harm. Let them drive but give them a map.

So, here's what the situation was. I was with my sister as she was in labor for support. Not at my hospital. Not there as a doctor. In fact, she pretty much told me to just be her sister and not argue about best practices with her doctors as I'm somewhat known to do...So I really was doing my best to keep my mouth shut. My sister, for context, is 29 and has type 1 diabetes. Labor wasn't going well and she wasn't progressing at all, so the hospitalist finally made a c-section call (about 2 hours when he should have, IMO). I have to say, I have some bias here because this doctor was also texting whenever he was in the room, spoke more to my BIL than my sister, and hardly looked at her at all. This is not her first baby, but this delivery was roughhhh. So, he makes the c-section call and my sister asks if he can do a tubal after the c-section. Now, my sister requested sterilization in the case of a c-section as soon as they found out they were expecting again because medically, her pregnancies are dangerous and complicated. The hospitalist said, "I don't see that in your chart, and I don't think you should make that decision in labor." Okay, fair enough, but as all three of us told him, "No, this has been the plan for 7 months. It's in the chart. It's listed under EVERY visit summary." Seriously. We checked afterwards. No way to miss it in her MFM notes. But he goes on this long-winded reply about how my sister and her husband are so young and people who are under 30 and get sterilized usually regret it and he thinks they should reconsider. They say no. They want a tubal. He gives more excuses (still texting, btw). But he agrees to do it if they insist. They insist. He keeps giving more excuses. Now, at this point, remember, my sister has been pushing for a few hours with no progress and is sobbing in pain. But this hospitalist just kept hmming and hawing when they'd insist on a tubal. Finally, my sister and BIl give up on the tubal just so he'll get her into surgery. Seriously. This was like 30 minutes of him texting and giving excuses for not wanting to do a tubal.

I would think it’s not an OBs place to “advocate” one way or the other. Give facts that are relevant to the persons scenario and answer their questions honestly. If sterilization is in their best interest due to a medical condition explain that. Offer other long acting birth control if they are not interested in sterilization.

I would think that advocacy is an essential part of any clinicians responsibilities. Advocacy takes many shapes and forms- None of which involve manipulating patients decision making to mirror our own personal biases (not sure if that is was had happened in this particular scenario or not).

Hello, I work at company that does early intervention for kids with ASD! I got prompted into being manger/coordinator role where my salary was increase to $65,000 and I accepted it and now I feel like I’m getting underpaid for the extra work I have to do. I’ve looked online and ranger for coordinator role is $75,000-$85,000 depending on experience. I have two years experience now being in my new role.
