The new company will call the old company to verify employment history most likely. And old company likely has a policy to only say start date and end date, with nothing on quality. Too risky to say anything that could be considered subjective.
There is also a database that payroll companies update with your paystubs. Some companies don't update it such as trinet. If it's not in the database then the future employer can call or you can provide paystubs.
Nope. I never bring it up unless they ask. It might depend as well if your company policy requires you to disclose it to determine if there's a possible conflict of interest.
