You should always put your best foot forward without any hesitation. We’re just recovering from a bad year and I’m sure not everyone is on top of their game. Make sure to take some time off, so you can bounce back to your normal self. Good luck!
Agree that you should put your best foot forward but be careful not to appear disconnected from reality/ arrogant. Talk about your strengths, but also go in with a plan to address how you will fix the potential shortcomings in case asked. Avoid the “I’m a gift to this world and you should feel honored to Promote me” type thing, which I’ve actually seen. People are human and more than being perfect it’s about showing you can recover/ course correct and still perform at a top level.

Present your best case. If they have any contrary feedback, let them be the one who brings it up. Don’t self sabotage
Question really is what would you hope to achieve in the discussion by sharing it? If you are looking to have your role restructured then it makes sense, orherwise maybe not unless he or she brings up a performance issue.
I think it depends on your manager. If they are sharp and care about you, they probably already noticed your downswing and would want to know how you’re really feeling and how they can help. Depends also on the culture of your firm if mental health is something people talk about

Think about how much you want to share. You don’t want to bias them for future actions. As for the conversation, acknowledge current poor performance but think of pivoting conversation quickly to what can you do turn things around and what support you need from your manager to help you succeed
I’m kind of in the same boat as you and it’s been taking me a while to get that work done (longer than it would’ve otherwise). I let my manager give me the constructive criticism that I need to work faster and just accepted the feedback graciously and told them I appreciated them being honest with me and wanting to help me improve. They said they would still support my promotion to consultant
