I FINALLY have a start date for IVF STIMs 11/5 and my company is requiring us to RTO on 11/1. I’ve expressed concerns and have asked to extend WFH for a period of time due to medical necessity (if I get sick, the cycle is cancelled and I’m out $$$$ and my sanity) but HR is asking me and my doctor to fill out forms that essentially say I’m unwilling to get the vaccine, which isn’t true. Does anyone have advice on how to handle? They’re saying this is the only form that’s applicable.
