I would talk to HR and just generalize it as health reasons if they ask. It's really none of their business so you don't have to give details.

I agree. ^ I was dealing with some issues and needed to take some time off- I spoke to my PPO about my situation and they offered me the leave of absence. Health is more important than anything and you're human, don't feel bad for what you feel :)
Just say you've been tired and want to do something to recharge yourself. Don't say the details, they don't need to know that and if it's medical related I'm sure they can't even ask you the details.

Been very depressed for a long time now and I think I just need some time away from everything. I don't really know how you're supposed to bring that up. I'm pretty sure it's not kosher to say that you are extremely depressed and need a break.
Do you get paid for medical leave ?
@OP talk to a doctor and see if you can qualify for medical leave based on depression, you may qualify for some time off and get paid for it by disability insurance. Worth a shot. Plus won't be any possible judgement if it's coming from your doctor, and it's handled (at least at PwC) by 3rd party
