How long do you have? Personally, why spend time in Milan when there are better places in Italy and it’s not that close to the Amalfi coast.

Totally agree with this (unless you want to spend some time in Lake Como, which you can train to out of Milan).
Really need to know how long you have to give input.
Both Spain and Italy are easily 2+ weeks, I would recommend pick one country and really explore it. That way you don't have to worry about additional flights and don't feel pressured to cram or rush. I would recommend Italy for a first-timer, since you can easily spend 3+ weeks in Italy, and it has everything from mountains to beaches
Travel time between Milan and Amalfi is long, unless you fly. Here’s my take: choose one. You can do
1) milan (2 days with a day trip to lake como”
2) train from milan to Venice
3) stay in Venice for 2 nights max
4) Venice to Florence (3 days in Florence)
OR
1) fly into Rome, spend 2 days in Rome
2) train down to Naples- don’t stay in Naples- get on the local train or take a pre arranged car service to amalfi coast. Don’t stay in positano- too many tourists. Opt for Amalfi town or ravello. From Amalfi/ sorrento you can take a ferry to capri for a day trip or stay overnight.
3) spend day or night on capri.. do the chair lift in anacapri for the best views.
4) ferry back to Naples from capri, and you can fly out of Naples or take the train back up to Rome.
Trains are super easy, but tickets in advance you’ll save lots of $$ that way.

Start with where you are interested. France, UK, Germany, etc. are all very interesting too, but no need to skip over Spain or Italy.
My recommendation for Spain is to definitely include Seville and Granada. Would recommend Florence and Rome obviously for Italy, skip Venice.

Depends on how long your trip is......There are A LOT OF places definitely worth visiting. If this is your first time, I would say focusing on the cities that you have high interest first if you have 2 to 3 weeks. I went to Europe 2-3 times a year for a few years now, still haven't finished them yet.
