I think it's totally up to you. If you want to introduce yourself as Dr., then I think it makes sense to refer to other people by their titles until they tell you not to. But if you are set on going by your first name, I think that's totally fine, but I guess I'd still call other doctors Dr. until they tell you not to worry about it as a sign of resepct
I've only been practicing for 7 years and that's including residency, but I've encountered both.
My dentist that I've seen for like 10 years now, used to just address me as "Carl" (made up name). But once I graduated med school and started residency he called me Dr. Sanders.I remember the first time he called me Dr. Sanders, I told him, "oh it's okay you can just call me Carl haha" and he replied with, "Oh no! Of course not! You're Dr. Sanders, you earned that degree and that title!" and ever since then he still calls me Dr. Sanders...
And on the opposite end, I was seeing my PCP and he called me Carl but I addressed him as, Dr. Baker. And later on in the appointment I called him Dr. Baker again, and he said, "Oh it's okay Carl, you can call me Alex."
So, I think what really matters is that you are respectful, which would be addressing your patients as "Dr. so and so". And if you are their doctor providing them care then you should introduce yourself as Dr. Your last name.
