I’d recommend a semi-private boat trip to Capri (private if you can afford it) that takes you to all the grottoes and drops you for a few hours to explore Capri and Anacapri
Spend a a day & overnight night with f possible, on Ischia. Take the Ferry from either Naples or Sorrento. Visit a Thermal park, Negombo or Poisidon. Walk to village of Sant’Angelo. Visit Giardini La Mortella gardens. Hike in the mountains. Do a winery tour.
Visit Ravello. the town, Villas, and views are beautiful.
Take a guided tour of Pompeii and Herculaneum. If you’re spending time in Sorrento, do a kayaking tour of Bagni Della Regina Giovanna. It’s gorgeous.
Enjoy sunsets on rooftop bars with views of Mt Vesuvius.
I haven't traveled much in that area, but Pompeii is not to be missed. Assume that's already on your list.
Also look up B&B Mama Rosa Positano. I haven't been there, but ran across it online years ago and have been wanting to include it in a future trip.
If you’re in Naples proper for more than a day make sure you do a tour of the sotterranea, it’s a very cool piece of history. Ideally you’re staying in one of the grand hotels, if not find one with a bar or rooftop and enjoy. Pompeii I agree cannot be missed, we really really enjoyed it, there is currently a live excavation site too, you can’t take photos on it but it’s really well preserved and I would go back just to see it again.
Hire a private guide for Pompeii and also do Herculaneum in the same day.
When in Amalfi, highly recommend chartering a private boat to take you around and do the grottos etc. Once in a lifetime experience and so worth it.
We stayed in Sorrento — it’s a bigger town and you can spend a few hours walking around. We enjoyed Ravello, walking up to Mt. Vesuvius (great views), and Pompeii. We didn’t spend any time in Naples and I didn’t feel like I missed out when we went to the Naples train station.
