I know of people who’ve stepped away for caregiving. It’s a perfectly normal rationale. Wishing you and your family the best in the time ahead.
If you know you want to leave, can you just be slow to ramp down while supporting your parent? Not sure your financial situation, but finding an in-house job can take awhile and I think firms have more flexibility (I know that sounds weird but small in-house teams can be very busy). If you are on your way out and have a sick parent then I think you can excuse not being a top performer.

Does your firm allow sabbaticals for senior employees? That way you could take a year to help your parent and then look for in house jobs when you return to your firm.

I would imagine that being honest about your career gap would go a long way. As an aside, I had a 7 month gap (early in my career) and literally no one has ever asked about it.
