Depends on what the firm negotiates with the lateral partner, but as far as I have seen it not unusual.

Not at our firm. If you were coming on as a new partner, and there is no dire need at the firm for your services or persona, you should expect to pay your own way.
We do not. When I left a former firm I paid for a tail policy myself and it was not onerous. I did speak to a firm (that I did not join) that offered to pay for a tail policy.
I paid for my own, but for me it was nominal
