Can someone explain how Tier ratings work at Kolkata AC?

How do I make out by reading snapshot what my Tier is? I got most - at grade, does that mean Tier 3/4? Coach keeps saying snapshot and Tier are different, then what is snapshot and what is Tier rating?

I asked the same question to coach multiple times, but never got any clear answer.

I am really biased toward people with clinical/in hospital experience because, quite frankly, healthcare is a different beast - lots of hand holding and change management.
