This usually happens when performance objectives aren’t clearly stated. I always start with “these are the tasks/projects I expect to be done” and then follow up with “and this is how I expect them to be done.” I have been in your shoes before and wrote out my personal versions of these statements and then reviewed them with my manager. I learned that I was making improvements in all the things ai thought mattered but there were other unspoken/undocumented expectations my manager had. I learned the manager was under extra pressure so he needed me to step it up in these new areas. I did that and also took a few extra smaller projects off his plate and that turned it all around.

how did you figure out the unspoken/undocumented expectations from your manager?
I think you have to be diligent in asking questions in this kind of situation. Explain to them that you can't fulfill expectations if you don't understand what the expectations are. Ask for written performance goals that they want you to achieve and make sure you have complete clarity, otherwise they'll keep pinning it on you
