I'll only do it if it's been 6+ months at a minimum between postings. Otherwise, I assume it's a ghost listing and thus a waste of my time. As far as cover letters go, YMMV. I don't think they're necessarily make or break, but if an application gives you the opportunity to submit one, you should. Never know, it might end up being the difference.
I don't think I've ever applied to the same job twice. If I've applied and it went nowhere, that's it, I'm done. As for cover letters, they can be effective if you know what you're doing. The idea is not to repeat your resume in prose form, but rather to highlight things that may not be obvious from your resume. It's your opportunity to show some personality and your skill at communicating. If you can write a good cover letter you shouldn't pass up the chance to do it.
