Not excessive. Policies are always changing. It's important to keep up to date.
That seems reasonable to me. Especially depending on the job and the industry. Things change quickly and it's good to get those changes reflected on paper so employees understand and reference those changes.
I've never had to do it here, that's not part of my official purview. At my old job I think I only did it once in 3 years, and that was very minor editing.
That doesn’t sound excessive at all. We operate in multiple states so I’m constantly updating ours due to individual state regulatory changes.

No I wouldn’t say that’s excessive, it’s something I’d consider that needs to be done annually, but really there isn’t many / if any adjustments needed.
I’m trying to get my company on an annual review basis.
