Once they want you, you can negotiate what you need just like any other “benefit”. Anyone who is human would understand why you waited to share incredibly personal health needs. Anyone who is not, well, they probably wouldn’t be supportive of your true health needs anyway.

You do not have to disclose your diagnosis which I am very sorry to hear. One thing I have learned there is no such thing as fair in corporate America. Though, good people have good intentions to do right by their employer will only find later they could care less. This is your personal journey.
It’s better if you do not discuss. Negotiate your needs. You do. It owe anyone an explanation why. If the need arises once you are hired then discuss. Please familiarize yourself with the FMLA laws.
You do not need to disclose medical with an employer and I'd recommend not doing so.
