I got a call from HR and offer discussion was over couple of weeks back and still waiting for them to send me final mail to release offer. May I know if they take this much time to release offer, when I did a follow up call after 10 days they said they are waiting for some approval to release offer.

You haven’t got any raises, or you just haven’t got a large raise? Doing your job well is doing the job they hired you for.
To me it sound like someone who isn't interested, or doesn't have time to really invest in helping you develop. Almost like a quick response so they can get back to whatever else they are doing.
Next time they say something like that l, ask them to be more specific. What kind of things would prove that you're performing to justify a raise? Get specifics. Be positive and persistent.
If they can't tell you any specifics, they are just going on vibes. Which isn't the best measurement.
After two years with a 0% raise, I would be asking specifics like what do I need to do and when can this happen by. If they can't answer those questions, or keep pushing it off start looking.
To be fair some people may not have the skill of knowing how to develop and evaluate people. They should if they're in that position. Asking these questions can help to move them in the right direction.
I concur with EY 1. Ask them to clarify by proving what specifically and or what have you NOT proven in the current role. If they still waffle then you will get your answer. Then you look for a new job elsewhere.

They are stalling you. Talk to your HR BP.
But not in a complaint way, but in a way to request clarity of career path an its connection to the compensation .
Have you received any raise or merit increase in pay since you have been at your current role?
After two years in a role should be more than enough time for you to "prove yourself" in your role. Has your manager mentioned anything on what you need to improve or offered any support to help you improve?
If your manager hasn't said anything about what you need to improve or is being vague on your goals, then it's time to start looking for a new role outside of your current company.
