The first trimester is an absolute hell regardless. I’m doing great second trimester. I am on Wellbutrin which is known to help with ADHD. I need it for depression but I think it’s definitely providing a bit of support in terms of attention.
But seriously, once you’re out of the first trimester I’m almost certain you’re going to feel better and have better concentration and be able to work a decent amount. The first trimester fatigue is unreal.
I came off mine for IVF, pregnancy and BFing. I struggled big time during IVF, but it was a little better during 1st tri and a lot better during 2nd and 3rd tri. Now that I’m doing a gradual wean at 10 months pp I am bigtime on the struggle bus again. Torn about going back on my adhd meds or not because we want to try for #2 in a couple months so I’d have to go right back off them to start IVF again. TBH I’m just surviving and trying not to worry that I’m using up all built up good will/social capital at my current firm.
Hope things get better for you in 2nd tri! I wish I’d been more mentally prepared for the hormone drop of stopping BFing, that has been the hardest part with my adhd.
Everyone has to do their own risk assessment, but I’ll add that before we had to go down the IVF route my plan (with my OB’s approval) had been to drop to a lower dose but stay on my meds M-F while pregnant. IVF plus AMA added some other incremental risks that made me less comfortable staying on them but whether to stay off or go back on them was a decision that was reassessed at multiple points in my pregnancy and will be the same if/when pregnant with #2.
I went back on my ADHD meds at 18 weeks at my doctor’s suggestion. There’s no way I would have been able to work the entire pregnancy otherwise.

Oat extract, DMAE, omega 3s, and magnesium are also known to treat ADHD. Maybe ask your doctor if they’re pregnancy-safe. I took magnesium and omega-3s throughout pregnancy.
Unfortunately, I found brain fog to be a pretty significant pregnancy symptom that I just had to push through. I would have chosen to take a leave of absence from my job rather than take stimulants while pregnant.
I stayed on my meds throughout - I worked with a perinatal psych and MFM before I even got pregnant and everyone decided that was best. I halved my dose at the beginning (didn’t have to it was my choice) but was suffering from really bad anxiety/depressive symptoms from being so scattered so they encouraged me to go back on my full dose. Even with that, however, I never felt like I was working at top capacity. Brain fog is real. Can’t imagine having gone off totally.
I take Wellbutrin and my doctor told me to continue. I am now in my third trimester (due in like 4 weeks) and I’ve been energized, focused, and alert since first trimester. Initially I stopped and it was not good for me. I felt depressed and had no motivation because I thought I had to stop. Once I mentioned it to my doctor, she stated that I should get back on them and stay on them. Baby has been thriving, growing, and healthy.
