If he likes tattoos and that’s the only thing that really resonates why not see if there is a temporary tattoo or henna studio near you? I’m in Chicago and there’s a studio that specializes in flash tattoos that last 30-90 days and fade completely.
Non-tattoo, unhinged ideas for teens: all nighter family night- midnight movie, late night drive fast food through, “drinking game” competition (think beer pong and flip cup but with soda); fortnight Olympics; favorite things day; movie marathon day with themed snacks and meals… I just scrolled Pinterest as well and there’s lots of great ideas there

Some ideas - local amusement park, professional baseball game, bowling/mini-golf/arcade, go for a hike, go to an indoor climbing wall, waterpark. I mean, my kid likes capybaras and I just found out there's a rescue an hour from our house where you can go cuddle them, so I know what we're doing for a special birthday surprise. But I'd suggest anything that's spending time together.
