Am a physician who has 2 kids and need to be at home to take care of them. Any ideas on what kind of work a physician with an MD and no other post grad qualification can do 100% remotely?

If you at least do an intern year you can apply for MD license. Unlicensed MD has few job options.

depending on your state, you could be a collaborator for nurse practitioners. Many companies. Like urgent cares just need a physician on call for collaborating on things that fall out of their scope. (I've always worked with a collab not on site-- some jobs not even in my state.) but it's still nice to have someone with a higher level of training available to bounce ideas off of. I believe som of the larger urgent care companies have these type of roles.

You might want to try healthcare IT. Lots of EMR companies and HC analytics companies hire physicians as SMEs

Yes, get at least 2 years post grad. Most places want at a minimum of 2 year requirement now.
And as far as I know neither MD or DO have a one year grand father in anymore.
Do it now, don’t stop or you will put it off too long. The kids grow up, get their own education and then off they go. You’ll sit there wishing you had done this or that! Trust me, I know
