Those are good questions to keep in mind. The problem is that the answers to most of them can only be ascertained once you're in position for a while. Discovering something like a toxic environment is often like the tale of the frog in the slowly heating pot of water. It's takes time before you realized you're cooked.

These are all important questions. The difficult part is being able to answer them with certainty prior to working there. You've got to do your research and read between the lines while interviewing to try to answer them.
From brief observation- do the current employees seem happy ? /stressed/ glum/ harried/friendly/stick stuck up...?
That would be great if we could answer those questions at an interview most if the time you cant. But what you can do is to listen, watch facial expressions and body language. If more than 1 person is interviewing look at how they play off of each other. That recently happened to me after leaving i giggled a little bit because I knew I wasn't getting the position.
