Background check companies only look at whether what you put on the form you submit to them is the same as what they find in their research. They don’t care what it is, just that they match. They will call your schools, employers, etc. You don’t want anything to be different as they will flag it and delay. Your hiring manager just gets report whether you cleared or not.

In a dilemma, I was a contractor for 2.5 years for an org. I got in as an engineer but upgraded my responsibilities to senior management consulting 6 months in the contract. my boss at the time didn't push for title update and I was happy with the salary and the situation.
Now after leaving the org, my title would show the old contract title (I imagine) on the employment history check and not helping me applying for senior consulting roles.
Title is less important than what you actually did. You can describe it in your resume and ask them to reach out to your old boss
They do. Depending on the county they’re pulling from, it can take a long time. Took 3 months for HireRight to get what they needed from Cook County in Chicago, so account for that when giving any notice (if you’re leaving for ACN).
