I made a pretty big move from a big branch to a startup. There were definitely some growing pains and expectation adjustments, but as long as you're patient and okay with the possibility of less structure, then why worry? I think the question to ask yourself is why you're considering a switch in the first place.
Any move from a large organization to a smaller one will likely require some adjustments. The lack of structure you mention can work both ways. It can feel like there's less bureaucracy, but it can also be problematic. There's just less support organization, there could be fewer resources, and so on. I went through something like that once and it wasn't a horrible experience, but it did at times require me to be flexible in my thinking.
