Yes, I have. Search the bowls, there’s lots of info in them with others’ shared experiences. Start with your doc and therapist, then tell HR you need to take medical leave (no need to share why; seriously, don’t tell them why). You’ll get the papers you need for FMLA and short-term disability pay, if it applies. And the process rolls from there. Congrats on taking care of yourself and best of luck in your healing.

Yes. I did this and I came back with no problems with my job status when I did. your job is protected once your leave us approved, so if you think your performance is affected and you may be in the line if fire because of that, do it immediately. This is a benefit to you, so don’t leave it on the table.
You will need a doctors note but it shouldn’t and doesn’t need to include any specifics about your diagnosis, just the accomodation recommendation which could include anything from working from home, a modified work schedule, or extended leave. They cannot ask what the reason is and you do not have to disclose. Look at your HR site for submission details.
I also suggest if you are feeling even kind if sort of burned out, electing for the highest leave coverage if you are able. It will give you a higher % of your income while youre on leave but drives your insurance amount per paycheck up a nominal amount.
One thing I do not know if what happens if anyone finds out you are working or getting paid by someone else while you are on leave unless you are taking unpaid leave so be careful if that is your plan.
Your job “may” be protected. If there is a job elimination, you may still be impacted. So not 100% guarantee but it’s more important to take advantage of your medical benefits and take the leave to take care of yourself! Recently I have considered this. But you truly have to be incapable of working to be eligible for leave of absence.
I have it, and I highly recommend it. I ended up quitting the same week I got back. Sometimes, that space is exactly what you need to help you move forward.
