I had a second-round interview today where the hiring manager spent the first ten minutes talking about their company’s family dynamic and how they work hard and play hard. Then, without missing a beat, they mentioned that the role requires being on-call during weekends without extra compensation. Family. Am I being too cynical, or does that word immediately make you want to withdraw your application too?

I immediately turn around when the recruiter or hiring manager starts talking about how the company is "like family". There isn't a more toxic group of people than someone's family and that's not something I want to spend all day in.

Is the role hourly or salaried? If it is salaried, there are some pros and cons as it works out to be less hourly pay, yet you are also eligible for bonuses and other perks that an hourly employee is not eligible for. If it is hourly your on call time could actually somehow be built into the higher hourly wage so it would look like you are not getting paid to be on call.
To me, work is work and if it were with an on-call rotation it would not bother me as first and foremost it is a job that pays plus, they did say that they work hard and play hard. Regardless of if I had a family or not, they would actually understand.

If you really can't work weekends because of other commitments than you should withdraw your application. How many hours are you expected to work since some companies expect all employees to work 50-60 hours a week without any additional compensation.
Have you read any reviews about this company on Glassdoor or on any other sites? You could also try to contact a current employee about their overall opinion of the company, Do they have a high turnover rate? Are they hiring since they are expanding or a company where most employee's quit because of the unrealistic expectations and overall stress.

I think that people like that are meaning they are going to play hard, whle you do the work. No extra compensation would be no for me, but I would come in for overtime pay.

Without overtime pay? They will suck the life out of you, literally. If you really need that job, take it but start looking for another one immediately.
