It depends on the company if it is a smaller company maybe you could negotiate. On a big company it wouldn’t make a difference for them and many people already opt out of insurance because they get it through their partner’s employer or other route.
Yes I have done this many times and it works. I'm not saying you're going to get a lot more money but it could be a little bit of leverage.

Don't think so, I've heard it can be easier negotiating higher benefit than higher cash pay.
Usually wishful thinking. If they actually do value the savings of their portion of premium, then they’d need to tie the pass through savings & related wage increase to you remaining off the policy, e.g. a stipend, bonus, etc; more hassle.
