Put medicine in a locked box w a key - no combo. Sounds like you have a smart child.
This is the answer. Plus always observing and having consequences for not following the rules. And keeping them busy with things they are allowed to do so they don’t get in trouble. My 1yo will drag a chair, climb it to grab a magnet from the fridge, then use it to unlock the magnet locks. She’ll also climb the chair to climb a console to reach the keys hanging on a hook. So keys also have to be somewhere out of reach. Our medicine is in the highest cabinet, highest shelf. She can’t reach that standing on a chair.

Magnetic lock on the cabinet worked for us. Just have to keep the magnet key out of reach/sight

How old are we talking? Just put it on a higher closet shelf if younger. Honestly medicine is one of those annoying things that you take daily vitamins etc and locking with keys somewhere else is possible but so inconvenient on the day to day.
How about installing a shelf on the wall about 6 feet high so you can reach it but no way they can? You can even use one of those command picture shelves or caddies - no nails needed.
