Once in awhile is fine with me, we all have those unpresentable days. But, anecdotal evidence alert, the ones on my team who are always camera-off are also the ones who most often answer a direct question with “can you repeat that?”
Funny enough, I did that in an in-person meeting today as I was working on a presentation.
I will happily have my camera on for every meeting. But I’m never coming in.
Yes. We are in a communications industry and body language is a form of communication. You’re still working. So get presentable. It’s not that hard. You don’t have to look amazing. Just not in pajamas. It’s not a high bar. Turn the chair so you have a wall behind you. FFS. Just show up in the most basic way.

I think asking people to have their camera on is pretty reasonable in a small meeting setting. If it's something big like an all hands or something then I get being annoyed, but seeing people's actual faces is crucial to collaboration and communicating well imo.

Being ‘presentable’ enough to not be ashamed to be seen by other human beings seems kind of the bare minimum level of professionalism to bring to a work day.

You can blur your background or use a virtual one? While I have some sympathy, would you previously have taken a day off work if you didn’t look presentable? I do think if we want the flexibility of remote/hybrid, there still need to be some basic principles of what attendance at work is. For me it’s a courtesy to others to let them know the meeting has your attention and you value their time too.
This is a different problem from not looking presentable. If you’re working, you’re not giving the other meeting participants the attention they are giving you. Talk to your account/PM team - either you can’t do the meeting, or the work you’re doing is delivered later. It’s unreasonable to expect you to do both, and you’ll probably be doing neither as well as you could do by juggling.
