I think it depends on your insurance but for me I didn’t need prescription to use the infertility benefit. I just had to enroll into the fertility benefit program, which is separate program from our health insurance. It was free of charge to enroll.
Was everything covered (cryopreservation, storage, ICSI, etc.) or only the “IVF package?”
No, the subsidy is for elective procedures. If you are diagnosed with fertility, it’s covered under insurance. At least for some plans; I’m not sure about all.
The 25 plus 15 is the insurance lifetime limit, at least for the United PPO
And to be clear, by subsidy I mean the social cryopreservation benefit
Just to pile on, the social cryopreservation is to freeze things, and this is reimbursed by Deloitte directly. No medical diagnosis needed, you pay out of pocket for meds and doctors, you put all the bills together and send to the email in the benefit. It’s max $20k at 50% reimbursement, so to get the full benefit you’re paying $40k over a lifetime. Depending on where you go, you could do 2-3 (maybe more if you go to CNY) retrievals (if you’re a woman).
The medical coverage is plan specific. You need to follow whatever the insurance provider says you need to do to get the benefit. That means enrolling in the fertility program, doing whatever annual tests, going to a network provider…
