It happens to all of us at one time or another. There's nothing you can do to change it, so there's no point beating yourself up over it. Review everything objectively to see if there were any red flags you may have missed, document them if so, and then move on. It also helps to remember how many times you've advocated for candidates and been right on the money. The fact you're employed tells me this is a one-off, not a recurring problem. You'd be out of a job fast if you were regularly hiring duds.
Yes unfortunately. I went out of my way to bring somebody on, and it immediately became apparent I had made a mistake. Just try and figure out what went wrong and learn from it.
It’s tough and I’ve been there. When I realized it, I made sure to focus on learning from the experience rather than blaming myself. We can only make decisions based on what we know at the time. I also remind myself that every hiring manager has one or two of these stories—it’s part of the job.

I never feel guilty. I might have recommended someone, but it's up to HR/hiring manager to finish vetting the person. It's not all on you.
