I very politely explain to them that as long as they are a high school student I don’t take friend requests because I have other teachers on my friends list and I don’t want it to be awkward for them. However, as soon as they walk across the stage with diploma in hand feel free to send me a friend request. Every student that I told this said they understood.
I have a firm "no current students" rule too, but once they graduate, I consider it case by case. Some students stay connected for networking or career advice, but I usually keep my privacy settings tight. If you're unsure, there's no harm in declining.

This is what I do. I always tell the. though that if I start teaching at their High School I will have to unfriend them. We usually have a laugh. I have received several in the past few years that I have accepted.
I work in elementary school so I don’t get students adding me but I do have parents try to add me. I just leave them and never add/dent so they can’t add me again. You don’t need to be a part of my social media unless you’re a friend or family member. I’ve never had anyone ask me why I didn’t add them, but if they did I’d just say that I like to keep my professional life and personal life separate!
Never. It is not worth the risk. Students are not friends.
