I’d call them for the initial ask or do it in person if possible
General structure:
Paragraph 1 - Thank them for agreeing to write your recommendation!
Paragraph 2: Explain why you want to go to business school, what are your post-MBA goals (ST and LT) and what do you expect to learn that will put you on a path to achieve your goals?
Paragraph 3: List of schools you are applying to with the deadlines (including time of day in the recommender's local time zone). And explain that you will put their contact information into the applications. They should get an email from each school with instructions.
Paragraph 4: Link to the GMAC Common Letter of Recommendation which essentially what EVERY program (now including WHARTON!) asks. This is a good starting place.
Paragraph 5: Examples that they can highlight about your strengths
Paragraph 6: Reminder of a time they gave you constructive feedback and how you addressed it.
Paragraph 7: Thanks again. Don't hesitate to reach out if you have questions. Note that you can't be involved in writing the recommendation but are available to answer questions.
