I’ve been the Skin and Wound go-to person on my floor for three years, and now I’m stuck doing every complex dressing change while my own patients wait. Management won't give me the title or pay of a Wound Care Specialist, but they expect the expertise. Should I stop being the helpful expert until they put it in my contract, or will that just burn bridges for future promotions?

If I found myself in your situation, I would feel it to be a sticky situation. I feel like I would need to go to management and sit down and say, "Listen, I'm doing all this extra work, and my other patients are suffering for it. Can you help me out?" But it sounds like you have a contract and that you would have no fear of retribution if you did that. So, if that's the case, I think I would just sit down and explain the situation to them, telling them that other patients are suffering because of you having to do every complex wound bandage.

Both of these are great answers. I know it's uncomfortable OP, but they are the ones who have put you in an uncomfortable situation and there is nothing wrong with having an honest conversation about it.

Have a conversation with management and address ur concerns— if they wont give you title or pay then ask them straight up what is needed? A certification they can help you pay for ? Some ER rooms now make all PAs do stitching for face- lips/ eyes etc vs trying to get a doctor or someone else like a plastic surgeon who are on call these days as you know. I visited an ER room recently and had a PA who was great at his job and an expert since he has progressed with stitching and mastered it for over 20 yrs— i was so nervous to have a PA do stitching vs a MD or plastic surgeon— he calmed me and told me— honestly if i call a dr if that is ehat u prefer they are going to tell me i should do the stitches bc they are too busy doing something else and know i can easily handle the task- this may hold true for you as well. You might be getting them because people know you are good at it! Your doctors and nurse colleagues— if you feel strongly about not doing something else many of them - still talk to someone about it. There are positives in this scenario and dont forget the immense skills you are getting for burn patients, surgeries, injuries , etc. not all bandage treatments are the same. Own that you are good at this!
