Whatever you select, get every ounce of insurance - you want a policy with a zero deductible. European car rental agencies will find every opportunity to charge you for “damage.”
With the zero deductible plans, you can bring the car back in pieces and there’s no charge.
In my experience, the US Credit Card insurance isn’t sufficient to deal with the local insanity in Europe.
For example, the loss of revenue charge from the car being out of service while it’s being repaired or the massive potential liability hold they place while the car is being fixed - up to 30k. It’s easier, in my experience, to just suck it up and pay for their overpriced insurance. And then absolutely not give a F about the car.
I have found that you can get pretty sizable discounts on the insurance by booking through the rental car companies’ apps and paying the balance due in advance - 40% or more.

Private companies do those. We got a charge for parking in a no parking zone - we stopped to turn left out of the property. You can fight them, but the rental car company charges a fee simply to forward the charge to you.

Consider whether you really need to rent a car. It’s extremely easy to get tickets in Italy, especially in Milan. They are expensive and complicated to settle later
