I am a new associate. I never know when to take initiative without stepping on toes or without asking for permission first. Has anyone ever felt this, and how have you dealt with it?

New grad. Starting at a small defense side med mal firm. Looking on information from anyone in this path on what the career trajectory is like for this line of work.

I'd like to move into a bigger firm after a few years. Am I stuck to med mal? Is it possible to change practices or am I pigeon holled. Why do partners tell associates to be cost-conscious when billing for certain clients when they can just cut your hours on the back end?
