Up the size
Make sure you get the fringe around the legs out of the diaper. There is a secondary sealing elastic inside. It's not about the tightness, more about the seal. And agree that it might be time to look at the next size up if you are getting blowouts up top. Also, consider that penises tend to point up, so there's only so much you can do.
What DD1 said about penises - tuck it down before sealing it up. Also, YMMV with disposable diaper brands, everyone's body is shaped differently - for me, Pampers worked well on son #1, Huggies not so much. Son #2, anything goes, so we saved a bundle by using the economy Luvs. When in doubt, size up if your child is big enough, height and width-wise - a bigger diaper absorbs more. The weight guidelines for diapers are simply that - a guideline. Nighttime leaks? That's what night time diapers are for (or premium diapers). Lastly, previous comments have focused on disposables - if using cloth, be prepared to change more often, that's the commonest solution.

Sure you’re using the right size and type? I bought pampers once and my wife scolded me saying they’re crap. She buys Huggies instead and these work great
Oh yeah - I use Kirkland diapers from Costco. Pampers didn't work for me.

Try a variety of diapers and see what works with your little one. Not all diapers work the same for all kids.
Size up or try different brands. Brand makes a difference. Seriously.
And when they are babies sometimes a blowout is going to happen no matter what you do.
