Forge it. It’s not like you’re lying. USCIS is not going to call your last employer and ask them about the letter. If they call them up at all, it’ll be to verify the duration of your employment and that will be true anyway.
In all the cases I have seen in past several years, letter from an old colleague/manager worked fine however, some attorneys want it notarized.
In my case an email from old colleague worked fine but recently, I gave the letter to one of my direct reportees and his firm wanted it notarized.

Yes . But definitely check with the current law firm who is filing PERM.
Its very common for example Wipro doesn't do it if you resign here in US. A silly and stupid way they think they are punishing people for resigning. Dont worry, get a experience letter signed by any of your old colleagues, it should be sufficient.
